Primary Care Visits
We conduct regular health assessments, preventive screenings, and wellness checks to proactively manage residents' health. We prescribe medications, order lab work, and perform diagnostic screenings.
Chronic Disease Management
We create tailored care plans for managing chronic conditions such as diabetes, hypertension, heart disease, and more, aiming to enhance residents' quality of life. Additionally, we offer advanced care planning and referrals for end-of-life care as needed.
Health Education
We provide residents and their families with educational resources and support to empower informed decision-making regarding their health and well-being. We also offer free education to assisted living facility staff.
Wound Care
We assess and diagnose a variety of wound care concerns, routinely evaluating wounds and managing treatment plans to ensure appropriate healing.
Senior Living Support
We assist with the completion of 602 Forms and are able to administer and interpret TB tests.
Acute Care
We promptly evaluate and treat acute illnesses and injuries, minimizing the need for hospital visits and promoting resident comfort.
Medication Management
We ensure accurate medication administration, monitor for adverse effects, and optimize medication regimens for improved outcomes.
Coordination of Care
We collaborate closely with facility staff, specialists, and other healthcare providers to ensure seamless coordination and continuity of care. Furthermore, we can provide referrals for physical, occupational, or speech therapy services.
Annual Resident & Employee Physicals
We perform physical exams for both residents and assisted living facility staff to meet facility requirements.
Consulting Services
Our consulting services for assisted living facilities extend beyond traditional care provisions. We offer comprehensive education and training programs tailored to the needs of facility leaders, including Executive Directors and Administrators, as well as nursing and care-giving staff.
These programs cover a wide range of topics, from regulatory compliance and policy development to best practices in resident care and medication management.
